Foothill HomeCare Partners provides non-medical in-home care across the San Gabriel Valley for seniors and others who are not ready to leave their home for an institutional setting, but because of illness or chronic conditions need support to remain at home.

Types of Care

Companion Care

Our caregivers help with all activities of daily living including, but not limited to, help with daily chores, preparing healthy meals, companionship, personal care, running errands.

Transitional Care

Our caregivers help with the often difficult transition from hospital/rehab/skilled nursing back to home. We also help transition our clients from home to assisted living or skilled care.

Specialized Care

Our caregivers provide care for more advanced needs with special requirements, such as dementia, Alzheimer's, stroke recovery, Parkinson's, feeding tube, hoyer lift and more.

Hospice Care

Our caregivers will your support your hospice company’s care plan, while focusing on quality and dignity of your loved one’s end of life transition.

Hourly Care

Our personalized and affordable services are available 7 days a week, 24 hours a day and can range from a minimum of 4 hours 1 day a week to 24 hours 7 days a week.

Insurance

In-home care can be paid for privately or with an existing long-term care insurance policy. Medical insurance and Medicare do not currently pay for or reimburse the cost of in-home companion care.

Activating a long-term care insurance policy can be difficult. We can assist with determining the ADL's (activities of daily living) necessary to get your policy started and help process the paperwork in a timely manner for a successful transition to LTC insurance paid care.
